Medical providers at Angelo State University cannot share clinical information with anyone, including parents, without the permission of student patients age 18 and older. The exception to this would be if students are in impending danger of hurting themselves or others, or are incapable of taking care of themselves.
ASU encourages students and parents to maintain open communication to avoid situations in which parents are worried about the health and well-being of their students.
A medical record is created when you receive treatment from a medical professional at Student Health Services (SHS). Each student's medical record is the physical property of SHS, and must remain in the custody of SHS staff at all times. Consistent care is exercised by all SHS staff to protect the privacy and confidentiality of patient health information. Medical information is not released by any member of the SHS staff without a student's consent, unless authorized by law.
Your medical records are the physical property of Student Health Services. The information contained in the medical record is confidential and will not be released without your permission, unless otherwise authorized by law. Patients can submit an Authorization for Release of Patient Information form to request access or copies of their medical records. Most requests for copies can be accommodated within 7 to 10 business days. Please contact us at least 24 hours in advance to make arrangements to view records or request copies for pickup. To help protect your privacy, you must present identification prior to viewing records or picking up copies from our office.
Medical information obtained from our website is not intended as a substitute for professional care. If you have or suspect you have a problem, you should consult a healthcare provider.
