Qualification for Care

To qualify for Home Health Services, you must be under the care of a licensed physician and you must be homebound (this is a Medicare requirement). You must also require skilled services, such as nursing or physical therapy. Home care is paid for by Medicare, Medicaid, commercial insurance, or self-pay.

Applying for Service

The Shannon office is open Monday to Friday, from 8 a.m. to 5 p.m. Registered nurses are available 24 hours a day, seven days a week for non-emergency care.
